A Needs Assessment Counselor will help determine which services or programs are appropriate for you or your child. Our Needs Assessment Counselors are master’s level therapists or RN’s that consult with Psychiatrists to help connect you with the necessary services or programs for growth and healing. If you are approved for the Partial Hospital Program (PHP), or Intensive Outpatient Program (IOP), one of our Psychiatrists will schedule an admission date for you. If you or your child meet criteria for a different level of care, our Needs Assessment Counselors will offer at least three referrals for the most appropriate services – or they can help you schedule an appointment in one of our clinics.
Admission to the PHP or IOP is designed to gather comprehensive information from various PrairieCare professionals to ensure accurate identification of problems and to create a realistic treatment plan. Our social worker will discuss obtaining releases of information (ROI) so we can talk to your other care providers. Information can only be shared with a valid ROI, or in emergency situations according to HIPAA standards.
Our social worker will schedule the admission date and time with you as soon as possible. Upon arrival, you will be asked to fill out intake paperwork. You will then meet with our social worker to complete a psychosocial assessment and then with our nurse to complete a nursing assessment. These two assessments may take 2 – 2.5 hours to complete. By the end of the first day, you will also meet with our Psychiatrist for a comprehensive psychiatric evaluation. At this point we will determine if further testing or assessment is needed, or it can be ordered later on during treatment as well. Our treatment team will create the treatment plan that will be reviewed during the first family meeting scheduled during week 1 of treatment. The treatment plan will then be reviewed and updated every week and can be discussed individually, or during your weekly family meeting. We also understand the urgency that surrounds accessing psychiatric care. We will schedule a tentative admission date at one of our sites, but we also reserve the right to change the date or site of admission.
