Not in the LPN’s scope of practice is the following: delegating, planning, directing, supervising, or evaluating the care of a patient. In the LPN scope of practice is the following: The "practice of practical nursing" means the performance for compensation or personal profit of any of those services in observing and caring for the ill, injured, or infirm, in applying counsel and procedure to safeguard life and health, in administering medication and treatment prescribed by a licensed health professional, which are commonly performed by licensed practical nurses and which require specialized knowledge and skill such as are taught or acquired in an approved school of practical nursing, but which do not require the specialized education, knowledge, and skill of a registered nurse.

Position Description: The Site Supervisor plans and manages care provided in PHP, Day treatment, IOP and other levels of care, as applicable, at the Edina site. Incumbent is responsible for ensuring staff is adequately oriented and trained, and quality standards are met in all areas. The Site Supervisor articulates and operationalizes the Prairie Vision, Mission and Philosophy; demonstrates knowledge of Psychiatric and Addictions treatment, demonstrates support for the program’s strategic plan; and provides leadership for establishing a program vision that is consistent with an organizational direction and goals. Helps maintain programs according to Prairie Standards of Care and standards of Joint Commission, CMS, state and federal regulations, EEOC and insurance contracts. The Site Supervisor is involved in outreach efforts and establishing positive relationships with referents and community providers. The Site Supervisor will meet regularly with the COO and Medical Director to review and coordinate program activities. May be asked to provide backup clinical coverage within the scope of his/her licensure and or provide internship supervisor within the same scope. The Site Supervisor is responsible to assess, plan, implement and evaluate the care of patients pursuant to the policies and procedures of Prairie. This includes triaging phone crisis phone calls, consulting with the on-call physician; coordinating admission and making appropriate referrals for care. The Site Supervisor is also responsible for acting as a liaison between the patient and third party payer to promote appropriate treatment for patients. The Site Supervisor will be required to extend supervision to other Twin Cities staff under similar or related licensure (i.e. RN for RN, LPN; LICSW for MSW, LMFT, etc) as needed.

 

Position Requirements: This position requires a minimum of a Master’s Degree with considerable healthcare experience, preferably in Psychiatric and Addictions treatment. The incumbent also has had previous supervisory experience. The employee must be able to effectively integrate services across disciplines and levels of care and to effectively communicate in written and verbal form with program staff, administrators, referents and the public. Additionally, the employee must have computer skills using Microsoft Office products including Outlook, Excel and Word.

Position Description: Responsible to assess, plan, implement, and evaluate the psychosocial services for patients of PrairieCare. The Therapist provides a comprehensive range of services to patients and their families and seeks to enhance the functioning of patients. Responsibilities of this position include the provision of group therapy, family therapy, and individual therapy for assigned patients. This position may work with children, adolescents, and adults and serving patients with mental illness, and/or a combination of both, as assigned. The therapist coordinates care with outpatient providers upon admission and discharge. The employee will be available to provide coverage for other Therapists when absent or as the need arises. The Therapist contributes to the interdisciplinary team the perspective of their unique training in the area of family systems and/or social work theory. The employee may be assigned supervision responsibilities for masters level family therapy interns, Masters level counseling interns, or masters level social work interns, based on qualifications. The employee is oriented and educated in gender, cultural differences, and age appropriate care for the assigned department.

This position requires a master's degree in social work, counseling, or a closely related social science or healthcare field and current licensure in the State of Minnesota as a Licensed Independent Clinical Social Worker (LICSW), Licensed Marriage and Family Therapist (LMFT), or Psychologist (PhD). Position Description: Responsible to assess, plan, implement, and evaluate the psychosocial services for patients at Prairie. The employee must have knowledge and understanding of Social Work practices and principles and be skilled in case psychosocial assessment and discharge planning. Expected to develop treatment plans, complete discharge plans and conduct discharge meeting may be assigned supervision responsibilities for BSW interns. Oriented and educated in gender, cultural differences and age appropriate care for the assigned department. Responsible for leading psychoeducational groups and communicating with patients and families to keep them involved in treatment plan decisions. Responsible for facilitating weekly treatment team meetings and creating and revising master treatment plans as agreed upon by the treatment team.

Requires a minimum of a Bachelor's Degree in Social Work and licensure in the State of Minnesota. Must have strong case management skills. Two years experience in a health care setting with adults preferred.

Oriented and educated in gender, cultural differences and in age appropriate care for the assigned unit. Plans and facilitates daily nursing groups with patients. Responsible for nursing assessment upon admission and the necessary discharge planning required in hospital programs. Must effectively track and communicate pertinent medical information both verbally and through written documentation. The RN can assist in scheduling appointments and medication refills. The RN supervises the LPNs and Psychiatric Technicians for the day to day operations. The RN can function independently and as the Charge Nurse.

Requires knowledge of nursing practices. Current licensure as an RN with the Minnesota State Board of Nursing is required. Required to be knowledgeable of and follow the Nurse Practice Act. Must have critical thinking skills, problem-solving ability and the ability to communicate and make safe judgment calls in stressful situations. Position Description : Responsible to assess, plan, implement, evaluate, and supervise nursing care of patients within the area of assignment pursuant to the policies and procedures of Prairie. Triages calls from the community, ER's, and/or families to see what services the patient should be referred to and if coming to PrairieCare is appropriate either for an assessment or a direct admission. Ability to assess medical stability and appropriateness of admission to PrairieCare programs based on medical needs. Ability to assess psychiatric symptoms and gather necessary information so that psychiatrist is able to determine with his/her input the proper level of care and disposition. Ability to assess the patient's acuity along with psychiatric and medical needs to see if Prairie Care has the capacity to meet those needs. Ability to make appropriate referrals for inpatient or outpatient services for a patient if his/her needs would be better met elsewhere. Responsible for gaining insurance pre-authorizations, when necessary, and conducts routine insurance reviews.

Oriented and educated in gender, cultural differences and in age appropriate care for psychiatric and chemical dependency services. Must have knowledge of DSM-IV criteria, possess interviewing and assessment skills and display a professional attitude in all aspects of his/her performance.

Ability to assist and cover on the inpatient unit as needed. Thus, be able to plans and facilitates nursing groups with patients. Responsible for nursing assessment upon admission and the necessary discharge planning required in hospital programs. Must effectively track and communicate pertinent medical information both verbally and through written documentation. The RN can assist in scheduling appointments. The RN supervises the LPNs and Psychiatric Technicians for the day to day operations. The RN can function independently and as the shift lead Nurse.

Must possess skills to communicate clearly and effectively with patients/families/referrants/co-workers as necessary to successful complete their job duties and to maintain a therapeutic and customer friendly environment.
