Needs Assessment

PrairieCare is proud to offer Free Needs Assessments to anybody looking for local psychiatric services.

A Needs Assessment is a face-to-face interview with a trained counselor to help determine which level of care is most appropriate for that person. Anybody can schedule a Needs Assessment at one of our three locations. PrairieCare can usually offer an assessment within 48 hours of initial contact.

A Needs Assessment is not a full psychiatric evaluation, but rather an assessment which determines the most appropriate level of care to address an individual’s needs. This may include, but is not limited to: outpatient clinic services, partial hospital, inpatient hospital, residential treatment, or substance abuse treatment.

To prepare for a Needs Assessment we ask that you be ready with the following:

  • Parent/guardian consent (if minor)
  • Custody paperwork/divorce decree (if applicable)
  • Contact information of past and current providers (we may ask to sign a release of information to communicate with these providers)
  • Any past clinical records
  • Insurance information (this is done to verify benefits to make an appropriate referral)

The following forms can be printed and completed beforehand. This will help decrease your wait time when you arrive for your appointment:

When approved for a service at PrairieCare, we will obtain a physicians order and schedule an admission. Please note that often times it is necessary to begin treatment as soon as possible, in particular if suicidal ideation or intentions are present.

This need to begin treatment immediately is complicated by the lack of psychiatric beds in Minnesota so unfortunately we are unable to guarantee services. For further information about this issue, please click here.
