PrairieCare Chaska is located at Two Twelve Medical Center. We work closely with local providers, and school districts to meet the demand for services in this area. Our campus in Chaska provides a Partial Hospitalization Program (PHP) for children and adolescents along with an Intensive Outpatient Program (IOP) for adults. Additional outpatient services include psychiatry, individual therapy, family therapy, and psychological assessment.
