Release of Information Forms (ROIs)
This form will provide PrairieCare with a document for your consent to share information about you and your care.
Please select the document that best fits your need from the options below, download, and complete all of the fields on the form. If you are unsure, please complete the standard form. Once you have completed your form, save it to your desktop.
Then start a new email to firstname.lastname@example.org, and attach your completed form. You can also fax this form to 763.315.0249.
Upon receipt, our PrairieCare team will review, and work to get back to you with a phone call. During this call, we will review your information and confirm what you need.
If these options are not suitable, you can drop off your completed form at the PrairieCare location nearest to you.
No Cost Mental Health Screen
If you are unsure of where to turn, have questions, or looking to schedule an appointment, please call our Assessment and Intake Team at: 952-826-8475.
Our team can help determine what type of care may be best for you, at 952-826-8475 for a no cost screening. This service is offered via phone between 7:00 am and 6:30 pm daily. If you are reaching out outside of these hours we can still talk with you about next steps and accessing services. Please send a request via email, link below, and you can expect a phone call from our Intake Advisor Team Members the next business day.

Schedule An Appointment
You can schedule a first time appointment by calling PrairieCare at 952-826-8475. We strive to respond to you within two (2) business days if you are not immediately connected with a team member when you call.
Given the intensity of the events in our community, and the world at large, we are experiencing an increased volume of individuals and families needing mental health support. As a result, a delay may occur in matching individuals to our services. We understand this may be frustrating or discouraging, and that this is not ideal.
We appreciate your patience as our team is working hard to return your message in a timely manner as we all navigate this time of increased need and concern. We assure you PrairieCare is here to provide the necessary healing space for all.
Prescriptions & Medications
For questions about your medications please contact your pharmacy or PrairieCare directly.
If you need a prescription refill, please contact your pharmacy and they will send a formal request to your provider. Please allow up to 72 hours for your refill request to be processed.
The following are normally required in order to obtain refills without coming in for an appointment:
- Having a return appointment scheduled.
- Keeping scheduled appointments.
- Having been seen within 3 months (children) or 6 months (adults) unless other plans are documented by your provider.
